PELAKSANAAN DISCHARGE PLANNING PADA PASIEN POST SECTIO CAESARIA

Abstract
The qualitative research with phenomenological approach
aims to investigate the implementation of Discharge Planning by officers
in the room Sakinah PKU Muhammadiyah Yogyakarta. Research
conducted on three key informants, namely the head of the room, the
midwife coordinator of the room, and patient. The data collected using
in-depth interview, content analysis, and observation. Analysis hypothesis
by triangulation method shows the implementation of discharge planning
is not perfect and there is no standardized format specifically for obstetric
patients. Discharge planning evaluation results showed 81% for
independence bathing the baby. Implementation constraints discharge
planning is the reluctance of patients, human resources not understand
the purpose of discharge planning and midwives were not involved in
planning the discharge of patients.
